How can I prepare for my office visit?

  • Please kindly bring your medications to all visits. You may choose to make a list if this is more convenient for you.

  • Please bring your insurance card and be prepared to pay any co-payment required by your insurance plan.

  • If you happen to arrive more than 15 minutes late to your visit, please note that you may be rescheduled for another time.

What is your cancellation and no-show policy?

If it is necessary to reschedule an appointment, please call us immediately. Unfortunately, please note that a $50 fee will be associated with appointments not kept or not cancelled at least 24 hours in advance of your appointment time. Please contact the office for more specific details.
